Copy of valid CNIC of Applicant.	Approved proposed and Completion Plan with QR Code, letter & Drawing from relevant authority, NOC from relevant Authority (where applicable)
Attested copy of property / ownership documents, in case of tenant, tenancy agreement along with landlord NOC	Original NOC from Society / relevant authority (if applicable)
Affidavits / Undertakings on Rs. 100/- stamp paper as per specimen(s)	Duly Filled-in RLNG Contract Form (also available on www.ssgc.com.pk)

I Solemnly affirm that:

SSGC Gas pipeline exists in front of my premises.
I fully understand that if the information given above as well as documents submitted is found contrary to the facts, SSGC will treat my application as invalid for any further consideration.
At present there is no dispute, litigation on above premises restraining Supply of Gas.
I clearly understand that all new domestic gas connections are currently available only under the RLNG tariff as per the directives of Government of Pakistan and I have no objection if Gas connection is provided under the OGRA notified RLNG Gas Tariff.

Cost of internal houseline, pipefitting & labor charges will be arranged and borne by the customer.
Customer will fill this form and submit along with above requisite documents and submit it to the relevant Sales Office / CFC Centre.
Please bring a photocopy of this duly filled form for acknowledgement receipt.
For more than one connection, separate forms will be required.
After submission of application, status of application will be updated through SMS on above given mobile no./or on SSGC Customer Connect App.
